Use this form if you have forgotten your MyData Portal account password.
Your password will be reset to a temporary password that you can use immediately to access MyData Portal. You will receive information about your temporary password on the screen after clicking the 'Submit' button.
A confirmation message will be sent to your district e-mail address. (You don't have to wait for the e-mail message to access your account.)
